ROLE


Be on a team! Work with your paired PCP to create surprise and delight for your mutually assigned patient panel (we keep those panels small too)

Promote Frontier culture of being intentionally relational with the personal care members receive in and out of your practice.

Customize the member experience by communicating directly with your patients by phone, text, and email whenever you want. No barriers!

Work collaboratively with colleagues to constantly improve and hone your craft.

Be a hero! Make home visits with your PCP for home bound patients, house calls for moms with babies, office visits for busy professionals.

Requirements:

Experience: Current unrestricted certificate/license.

You love people and are highly relational.

You expertly handle sensitive matters with integrity and a high degree of confidentiality.

You are well-spoken and articulate, with a dash of charm.

You communicate a plan and follow through to complete execution.

HERE ARE THE DETAILS


We believe in putting the right person in the right seat. Here's a little bit of the position you will fill, the day to day pulse of your work life at Frontier.

Know how DPC works (don't worry, we have an awesome training program)

Educate and empower members on how they can use their membership inside and outside of Frontier

Engage new sign ups and get them hooked on Frontier by taking proactive steps to schedule their first appointment

Create regular follow up to ensure members feel cared for and know that YOU have their back in the complicated medical system

Welcome all members at any point of contact. Healthcare can be scary, but your smiling face and attention to detail lets them know they are in good hands.

Answer phone calls, texts, emails etc. to schedule appointments, refill prescriptions, request medical records-whatever the member needs. Create surprise and delight.

Be a quarterback for your PCP - Your PCP calls the plays and you help carry the ball forward. Coordinating and anticipating physician needs related to patient care to set him or her up for success. Here is a list of a few of those items.
