MODALITY: PT REQUIRED SKILLS: 1+ Years experience, Active TX License # OF WEEKS: 13 SHIFT/HOURS: 8D, 8am-5pm

Licenses/Certifications: CPR/BLS (Basic Life Support), State License-Physical Therapist-Texas, NPI NUMBER
