Summary Coordinates the care needs of assigned patients and
develops comprehensive plans to manage care delivery across the
patient care continuum. Partners with patients and their primary
physicians to develop customized care plans based on their

in nursing. Preferred: Bachelor's or master's degree. Experience
Required: 2 years nursing experience. Certifications Required:
Currently licensed, certified, or registered to practice profession
as required by law, regulation in state of practice or policy. AHA
