group hiring - Location: LaPorte, Texas - Outpatient Quality Care /
Managed Care - In Office -M-F:8a-5p - No weekends - Low patient
panel - Great Work/Life balance - Comfortable with use of
Electronic Medical Records (EcW) - Detect and manage chronic
diseases - Develop patient strategy to reduce hospitalization and
ER visits - Collaborate with Case Managers and Specialty Providers
- Educate patients on the value of compliance - Compensation &
Benefits: - Competitive base salary of $230,000"$290,000 (depending
